Nutrition Program

In Mattapan Community Health Centers new demonstration kitchen, Community Nutritionist and Certified Diabetes Educator demonstrates how popular snacks can be prepared in a healthy manner.

The Nutrition Services at Mattapan Community Health Center provide individual diet counseling on weight control, diabetes, and other topics. It also provides nutrition care to patients and families. As an integral part of an interdisciplinary health care team, the nutritionist provides evaluations as well as ongoing treatment through individual and group settings. The Nutrition Program offers the following specific services:

  • Diabetes and weight management counseling
  • Prescription/specification of food and nutrient intake
  • Translation of nutrition requirements into meal plans, food choices, and preparation techniques
  • Enabling behavior change by educating, counseling, motivating, and advising
  • Referral to other service providers, when necessary

Diabetes We Care Program

Our diverse Diabetes WeCare Program is an on-going program which provides additional care and support for Mattapan Community health Center patients and their families in communities we serve.

The folowing services are offered:

  • Hands-on Diabetes Education and Case Management Services
  • Assistance with Insulin Teaching
  • One-on-one Diet Management and Healthy Eating Consultation
