Our Health Advocates are Volunteers and Professionals
Health advocacy includes services to the individual and family as well as activities that promote health and access to health care in our community. Our Advocates support and promote patient health and help build the capacity to improve community health. Our Patient representatives, care managers, patient navigators and health advisers are health advocates who work in patient care setting that include hospitals, community health centers, and long term care facilities. They collaborate with other health care providers to mediate conflict and facilitate positive change as well as empower the patient to have a role in managing their own health.
