Help us learn how life experiences affect our health and our community!

Mattapan Community Health Center is working with the Harvard T.H. Chan School of Public Health to conduct a research study called the Life + Health study that looks at how life experiences affect our health and the health of the community. Participants will complete a 2-part 60-minute survey, sharing thoughts and feelings about their lives, health, and health habits. After completing the study, participants will receive either a $40 Amazon, Whole Foods, Stop and Shop, Star Market, or America’s Food Basket gift card.

If you are interested in finding out if you are eligible for the Life + Health Study, please fill out our screener by clicking here: Life + Health Study

This study is for Mattapan Community Health Center patients between the ages of 25-64 who were also born in the United States.

To contact the study team, call 617.620.1321 or email thelifeandhealthstudy@hsph.harvard.edu. You can also check out our study webpage: www.hsph.harvard.edu/thelifeandhealthstudy
