Adult Medicine at Mattapan Community Health Center. A team model of care with a focus on wellness and prevention. Mattapan Community Health Center's Adult Medicine department provides outstanding comprehensive primary care and well care management to adults. Our knowledgeable staff of physicians and nurse practitioners will provide personalized quality care ranging from routine wellness visits to the treatment of acute and chronic illnesses. As part of managing your healthcare needs, we will coordinate specialty care within our health center or at any of Boston's top hospitals.

We offer:
Preventative health services, including cancer screening and health counseling
Chronic disease management, including management of diabetes, asthma, and hypertension
Acute care services via both scheduled and walk-in appointments as well as 24-hour phone access to providers

About The Adult Medicine Program at Mattapan Community Health Center
The Adult Medicine Program at Mattapan Community Health Center provides comprehensive primary care to adults aged 18 and older. This includes routine physicals, same day visits for urgent problems, preoperative physicals, and routine follow-up treatment of chronic problems such as asthma, diabetes, high blood pressure, and chronic heart and lung diseases.
As part of our routine medical exams, we provide:
screening for cancer 
hypertension and other chronic diseases
cholesterol screening
counseling on healthy lifestyles
We also help coordinate specialty referrals and home care when appropriate.
Each care team in Adult Medicine is led by a Medical Doctor along with Nurse Practitioners, Registered Nurses, Care Managers, and Medical Assistants working together to ensure seamless continuity. One of the major reasons we have care teams at the Health Center is so you will always meet with a provider who knows you. For example, if your primary care provider (PCP) is not available for an appointment with you, one of their team members, familiar with your health and treatment goals, will see you instead.
